Introduction

Minoxidil is the most widely used topical treatment for male pattern hair loss. Available as a solution or foam, it stimulates hair follicles and promotes regrowth — particularly effective at the crown area. Unlike finasteride (which requires a prescription), some minoxidil formulations are available over the counter in Singapore, though medical guidance ensures optimal results.

How Minoxidil Works

Minoxidil was originally developed as an oral blood pressure medication. Hair growth was discovered as a side effect, leading to the development of topical formulations for AGA. The precise mechanism of action is multifactorial:

  • Potassium channel opening — relaxes smooth muscle in scalp blood vessels, increasing blood flow to follicles
  • Growth factor stimulation — upregulates vascular endothelial growth factor (VEGF) and other growth factors
  • Prolongs anagen phase — extends the active growth phase of the hair cycle
  • Increases follicle size — reverses miniaturisation, producing thicker, more visible hairs[^1]

Application Guide

Proper application technique is essential for optimal results. Incorrect use is a common reason for perceived treatment failure.

Solution (5%)

  1. Part your hair to expose the thinning area
  2. Using the dropper, apply 1 ml directly to the scalp
  3. Spread evenly with fingertips
  4. Do not shampoo for at least 4 hours after application
  5. Wash hands thoroughly after application

Foam (5%)

  1. Dispense half a capful onto fingers
  2. Apply directly to dry scalp in thinning areas
  3. Massage gently into scalp
  4. Allow to dry completely before styling

Frequency

  • Standard: Twice daily (morning and evening)
  • Minimum effective: Once daily (may produce slightly reduced results)
  • Consistency is critical — irregular use significantly reduces effectiveness

Common Application Mistakes

  • Applying to wet hair (dilutes the product and reduces absorption)
  • Using too much product (more does not equal better results)
  • Applying to hair rather than scalp
  • Stopping during the initial shedding phase (this is actually a good sign)

Expected Results

Setting realistic expectations is important for treatment adherence over the long term.

Timeline

Month What to Expect
0–1 No visible change; possible increased shedding
1–3 Shedding stabilises; fine vellus hairs may appear
3–6 Visible new growth in responsive areas
6–12 Maximum regrowth; hairs thicken and mature
12+ Maintenance — continue use to retain benefits

Realistic Outcomes

  • Best results: Crown (vertex) area — minoxidil works best here
  • Moderate results: Mid-scalp thinning
  • Least responsive: Frontal hairline recession (finasteride is more effective here)
  • Individual variation: Response varies significantly between individuals; approximately 40–60% of men see meaningful regrowth[^1]

The Initial Shedding Phase

Many men experience increased hair shedding during the first 2–4 weeks of minoxidil use. This is called "dread shed" in online communities and causes many men to quit — which is a mistake.

Why It Happens

Minoxidil pushes resting (telogen) hairs out of the follicle to make way for new growth-phase (anagen) hairs. The shedding is temporary and is actually a sign that the treatment is working.

What to Do

  • Continue treatment — the shedding will stop within 4–6 weeks
  • Don't panic — the hairs being shed were approaching the end of their cycle anyway
  • Document your baseline — photos before starting treatment help you assess real progress

Side Effects

Minoxidil's side effects are generally mild and localised, since systemic absorption from topical application is limited.

Common Side Effects

  • Scalp irritation — redness, itching, dryness (more common with solution than foam)
  • Scalp dryness/flaking — the propylene glycol in the solution can cause dryness
  • Unwanted facial hair — if minoxidil transfers from pillows or hands to the face

Less Common

  • Headache — rare with topical application
  • Dizziness or lightheadedness — very rare; indicates significant systemic absorption
  • Heart palpitations — extremely rare with topical use; discontinue and seek medical attention

Irritation Solutions

  • Switch from solution to foam (foam contains no propylene glycol)
  • Reduce frequency to once daily
  • Use a gentle, fragrance-free shampoo
  • Ensure scalp is completely dry before application

FAQ

1. Can I use minoxidil without finasteride?

Yes, but results are better with both. Minoxidil alone can slow hair loss and promote regrowth, but it doesn't address the hormonal cause (DHT). For optimal results, combining both treatments is recommended.

2. Does minoxidil work on a completely bald scalp?

No. Minoxidil works by stimulating existing (even miniaturised) follicles. If the follicles are completely gone (as in long-standing baldness), minoxidil cannot regenerate them.

3. Can I use minoxidil on my beard?

Minoxidil is sometimes used off-label for beard enhancement. Some men report increased beard density, but this is not an approved indication and should be discussed with a doctor(Based on MOH guidelines and prescribing information).

4. What happens if I stop using minoxidil?

Gains are lost gradually over 3–6 months after discontinuation. The hairs that regrew due to minoxidil will miniaturise again without continued treatment.

5. Is minoxidil safe long-term?

Topical minoxidil has been used for over 30 years with a well-established safety profile. Long-term use is considered safe for most men. Regular use is required to maintain benefits.
